Position Title: Registered Nurse – Care Management, Long Term Care Facility

Qualifications:
Education/Training:
  • Graduate of accredited school of registered professional nursing with a current license to practice nursing in New York State.
  • Must meet eligibility criteria for NHTD/TBI and/or Care Management roles, as amended. UAS and PRI/screen certified.

Experience:
  • Minimum of 2 years’ experience as a Registered Nurse required. One or more years experience as RN in home care setting preferred.
  • Prior recent experience in care management or similar role; knowledge of existing services and referral processes for community-based services for individuals with disabilities or seniors.

Skills:
  • Strong written and verbal skill; personable, self-directed, able to work independently, highly motivated, goal oriented.
  • Highly responsible, creative and ability to act on independent judgment. Demonstrated ability to achieve targets and work in a collaborative environment.

Physical Demands:
Must be able to sit, stand, walk, bend, and crouch frequently throughout the day; must be able to lift a minimum of 50 lbs unassisted; must have the ability to fulfill the terms of the job description.

Other:
Must have valid NYS driver’s license; free of disqualifying offenses; reliable transportation capable of traveling in excess of two (2) hours at a time.
Primary Function: The primary function of the position is support the operations of the Care Management and Service Coordination program (Care Management); complete required assessments using applicable forms; providing pre-enrollment education on behalf of third party insurance programs; and assuming care management responsibilities as assigned.

Primary Duties and Functions:
  • Conduct in –home assessments using required forms and procedures, including but not limited to Universal Assessment, Patient Review Instrument, etc. in a timely manner.
  • Provide required reports on assessments, productivity, outcomes, etc. as requested.
  • Serve as designated Care Manager for clients as assigned; adhering to all program requirements that may exist based on program.
  • Maintain all required records, forms and documents according to policy, regulatory standards, etc.
  • Formulates individualized, realistic and measurable short-term goals in planning care. Involving client and/or family in this process.
  • Assesses client needs, appropriateness of care being provided, and changes as needed.
  • Observe and provide ongoing assessment of client as needed.
  • Work collaboratively at all times with other facility programs and staff to achieve appropriate utilization of services and to maintain positive working relationships.
  • Represent facility, as appropriate in the community-related events where name recognition and public exposure is desired.
  • Foster existing relationships and nurture new connections with community providers, health care providers, and insurers as appropriate.
  • Assure timely submission all required billing sheets, forms and codes promptly to assure proper and timely billing for services.
  • Actively participate in Quality Improvement Activities.
  • Performs related duties as required.

Secondary Duties & Responsibilities:
  • Provide support as necessary for other facility programs, primarily home and community-based services.
  • Participate in and support organization-wide quality improvement initiatives.
  • All other duties as assigned and as necessary for the efficient and proper functioning of Care Management.
