Locations: Capital District - Albany, Saratoga, Schenectady and North Country (New York)

What You Will Do (not limited to):
  • Deliver high-acuity nursing care to patients and residents across hospital, LTAC, doctor office, and rehabilitation environments.
  • Collaborate with floor nurses, physicians, and multidisciplinary teams (physical, occupational, and speech therapists) to support patient goals and outcomes.
  • Monitor progress, adjust care plans, and coordinate with the care team to promote optimal recovery.
  • Educate patients and families on post-discharge care, wellness, and self-management strategies.
  • Perform admission assessments and collect health histories from patients, residents, and/or significant others.
  • Communicate clearly and effectively with patients, families, and team members.
  • Utilize available resources to assess patients or residents with new, unfamiliar, or complex conditions.
  • Document care accurately and in compliance with regulatory and facility standards.

What You Will Need: Required:
  • Current RN license to practice in the State of New York.
  • Associate Degree in Nursing or Nursing Degree from an accredited school.

Preferred Qualifications (optional enhancements):
  • Proven clinical experience in hospitals, acute care, rehabilitation, or long-term care settings.
  • Strong assessment, critical thinking, and meticulous documentation skills.
  • Effective communication and collaboration with multidisciplinary teams.
  • Commitment to patient- and family-centered care.
